Job Description
Role & responsibilities:-
Assessment & Case Management
- Interview clients and families to evaluate their psychological & social stability.
- Develop individualized case plans with clear goals, actionable steps, and measurable timelines.
Advocacy & Resource Navigation
- Advocate for vulnerable clients within complex systems like courts, government agencies, and medical networks.
- Maintain an updated directory of local resources, community networks, and non-profit services.
Crisis Intervention & Counseling
- Deliver supportive counseling to help clients develop healthy coping mechanisms and problem-solving skills.
- Intervene in safety situations, filing mandatory reports for suspected abuse, neglect, or exploitation of children and vulnerable adults.
Administration & Compliance
- Maintain detailed case notes and client files in compliance with state regulations and agency standards.
- Protect client confidentiality in accordance with professional ethical codes and privacy laws.
Preferred candidate profile
Education- Bachelors Degree in Social Work (BSW) or a Masters Degree in Social Work (MSW) from an accredited institution.
Experience- Minimum 2 Year in Healthcare
