Clinical Pharmacist - Hospital Setting (On-Site, India)

Role Responsibilities

• Verify and dispense prescriptions, ensuring dosage accuracy, drug compatibility, and compliance with hospital formulary.
• Conduct medication reconciliation on admission, transfer, and discharge, documenting interventions in the Electronic Medical Record (EMR).
• Collaborate with multidisciplinary rounds to optimise pharmacotherapy, monitor therapeutic drug levels, and adjust regimens based on lab values.
• Lead antimicrobial stewardship audits, ADR reporting, and drug utilisation reviews to support quality-of-care metrics.
• Educate patients and caregivers on medication usage, side effects, and adherence strategies before discharge.
• Maintain inventory, cold-chain integrity, and controlled-substance records in alignment with NABH and CDSCO regulations.

Skills Qualifications

Must-Have

• B Pharm with valid PCI registration.
• 2+ years clinical or hospital pharmacy experience.
• Proficiency with EMR/e-Prescription and pharmacy information systems.
• Sound knowledge of pharmacokinetics, drug interactions, and therapeutic drug monitoring.
• Working familiarity with NABH standards, CDSCO, and state drug control regulations.

Preferred

• Certification in Antimicrobial Stewardship or Oncology Pharmacy.
• Experience conducting medication therapy management (MTM) programs.
• Ability to train interns and junior pharmacists.
