Diabetic Educator - General Medicine

Medcare Hospitals Medical Centres is looking for Diabetic Educator - General Medicine to join our dynamic team and embark on a rewarding career journey

1. Education and case management for adults who have Type 1 and Type 2 diabetes
2. Phone consultations with patients and other nurses, advanced practice nurses, physician assistants and physicians in the community
3. Determine appropriate options in resolving complicated inquiries and/or issues related to diabetes management
4. Coordinate research protocols involving insulins, glucose meters, insulin pumps and medications used to manage diabetes
5. Contribute to new and ongoing diabetes clinical research protocols
6. Charting/data entry in EMR
