Strong understanding of risk adjustment models, HEDIS, ACO measures
Ability to support data ingestion from EMR, HIE, claims, SDOH sources
Skills to validate cross system quality metrics, attribution, and outcomes
Familiarity with multi disciplinary care management workflows
Integration experience with ADT feeds, CCD/CCDA, FHIR APIs
Experience performing quality analytics for population level reporting
Experience working with Population Health systems like HealtheConnections, Population Health Patient Registry,Performance Pathways and other similar applications
